$175 Fee Proof of Citizenship ( birth certificate/photo pg of passport)  Signed Waiver Form*

Return this form with the $175 registration fee and proof of US citizenship (copy of birth certificate or picture page from passport) to US Field Hockey at the address below

*For applicants not currently members of US Field Hockey. All B camp participants must be members of US Field Hockey. To become a member, send a completed membership application with signed waiver and membership fee check with your B Camp registration form.
