Remember, you are making a commitment to be at the tournament where you select to be rated – you cannot do service at the same tournament.  This application must be received by the National Ratings Chair by DECEMBER 1.

This does not guarantee that you will be able to be rated at the tournament you select due to limited numbers of places at each tournament.  They are assigned on a FIRST COME, FIRST SERVE BASIS.  ON THE BACK OF THIS APPLICATION, CHECK THE LOCATION WHERE YOU WISH TO BE RATED.

REMEMBER – You are making a commitment to field hockey and to the USFHA in attempting to earn a Level I rating.  Consequently, you MUST give services to the national governing body to be eligible to renew a national rating.  You may not give service in the tournament in which you are being rated.  Service can be also be working on a national committee approved by the USFHA umpiring Executive Committee.  If service is not given, ratings can be denied to renewal candidates.
