• Applications must be received by May 12, 1999 to enter the course. Registration is limited to 25 participants. Walk-ins will be accepted only if the registration limit of 25 has not been reached.
• Participants will be responsible for their own food and housing. The Radisson Simi Valley is offering a room rate of $75 (single through quad occupancy). Reservations must be made by May 12 to guarantee the $75 room rate by calling 805-583-4000.
• A confirmation letter will be sent upon acceptance.
• Note: Non-members receive one year USFHA Coach membership as part of the course fee.
